Billing and Insurance Information
The Indiana University Department of Ophthalmology contracts with McKesson Technologies to submit all insurance claims for its patients. All co-pays and charges for refractions are due at the time of service. We cannot guarantee payment of a claim. Therefore, payment for services becomes the patient’s responsibility if there is no response from the patient’s insurance carrier. If patients do not have insurance coverage, payment is due at the time of service unless other arrangements have been made. We accept cash, check, Visa and MasterCard.
To assure maximum coverage from the insurance company, the patient should contact his/her carrier before the visit to obtain any necessary pre-certification or prior authorization. We require a photo ID and a current copy of a patient’s insurance card for each visit. A copy of the insurance referral form is also required, if applicable. Fitting of contact lenses for cosmetic purposes and eye refractions are almost always NOT covered by insurance policies. For specific coverage issues, a patient should contact his/her insurance carrier.
McKesson Technologies will file both primary and secondary claims. It is very important to identify primary and secondary insurance at the time of the appointment. It is also important to identify if there is a Medicare Replacement Plan and if Medicare is a secondary payer.
McKesson Technologies is trained to answer any calls concerning a patient’s account, questions regarding financial assistance, and payment arrangements. These calls should be made to their Customer Service Unit which will direct the calls to the appropriate claims specialists.
